Supports for Daily Living Resource Manual and Video: A New Vision of Assisted Living for Seniors
PSL was one of three Supportive Housing providers who provided leadership in the development of the SDL Model in collaboration with the MHLHIN in 2008-09. The other two agencies were Nucleus Independent Living and Oakville Senior Citizens Residence. Since that time we now have 8 SDL providers in total. Steve Kavanagh served as the chair of the SDL leadership team during the development phase of the model.
Last year, PSL was pleased to have been invited to serve on the SDL manual writing team to prepare the SDL Resource Manual and Video.
The Supports for Daily Living Resource Manualis a document designed to help organizations develop an effective SDL program in their community. By utilizing the Community Support Services sector as a change agent, these frail and vulnerable seniors can be cared for in the community with appropriate supports. The purpose of the manual is to provide Local Health Integration Networks and health service providers with an understanding of how the Supports for Daily Living model works, evidence of the positive impact the service can have on health system performance, and the background knowledge and guidelines to help develop this important service in your community.
