Referral Procedure for Mississauga Seniors
- Contact SDL Central Registry
- General public referrals or inquiries should be directed to SDL Central Registry: 905-281-4443.
- Waitlist and Prioritization
- Eligible referrals are placed on a waitlist prioritized based on:
- Level of need for SDL services
- Current usage of healthcare resources (e.g., recent hospital discharges are prioritized)
- Eligible referrals are placed on a waitlist prioritized based on:
- Assessment
- A detailed assessment may be conducted if one has not been completed in the last 6 months prior to admission.
- Referral Form Submission
- All healthcare professionals and community partners must use the standardized SDL Referral Form and submit it to SDL Central Registry.
- Peel Senior Link services use this form to ensure all referrals are processed correctly.
