Referral Procedure for Mississauga Seniors

1. Contact SDL Central Registry
   • General public referrals or inquiries should be directed to SDL Central Registry: 905-281-4443.
2. Waitlist and Prioritization
   • Eligible referrals are placed on a waitlist prioritized based on:
     • Level of need for SDL services
     • Current usage of healthcare resources (e.g., recent hospital discharges are prioritized)
3. Assessment
   • A detailed assessment may be conducted if one has not been completed in the last 6 months prior to admission.
4. Referral Form Submission
   • All healthcare professionals and community partners must use the standardized SDL Referral Form and submit it to SDL Central Registry.
   • Peel Senior Link services use this form to ensure all referrals are processed correctly.
