You must participate in an initial pre-assessment to help evaluate your needs and determine whether the Supports for Daily Living program is right for you. If you qualify, your name is placed on a waiting list for service.
When an opening becomes available a trained coordinator will come to your home and complete a standardized needs assessment. Once the standardized assessment is complete a customized home care plan that meets your specific needs will be developed. If your needs change, the site Supervisor can reassess your health and adjust your plan of care.
Referrals can be made one of three ways:
1. Patients currently in hospital
2. Clients currently receiving CCAC services
3. Clients in the community
The waitlist is re-prioritized on a needs basis, not on a first come first serve.
If you don’t qualify for government-funded services and believe that you should, you can appeal the LHIN decision through the Ontario Health Services Appeal and Review Board (HSARB). You can also contact the Board if a service you previously received has stopped or been reduced. Contact HSARB:416-327-8512 (Toronto area)1-866-282-2179 (toll-free).
