Safe, Person-Centred Medication Support
Since 2010, Peel Senior Link has partnered with Resident Care Pharmacy to provide medication management services for seniors in Mississauga and Brampton. This program supports clients on multiple medications or those who need help maintaining compliance, ensuring safety and independence at home.
How Our Medication Management Program Works
- Blister Pack Distribution: Resident Care Pharmacy provides pre-packaged medications to all 11 Peel Senior Link sites, kept in a secured environment.
- Medication Reminders: Clients who are cognitively able but may forget doses receive reminders either via phone or in-person.
- Medication Supervision: Clients requiring guidance or encouragement are assisted by a Personal Support Worker, who ensures medications are taken as prescribed during visits.
This approach ensures safe, accurate, and timely medication management, while supporting seniors’ independence and peace of mind for caregivers.
