Brampton (February 21, 2019) – Peel Senior Link (PSL) was a recipient of a honourary mention at the Central West LHIN Quality Awards 2019. There were a total of 15 applicants, of which 3 agencies received the honourary mention.
Peel Senior Link, in partnership with Resident Care Pharmacy, developed a comprehensive Medication Management Program (MMP) that reduces medication-related errors, hospitalization rates, delay or avoid long-term placements, and ensure that seniors have the capacity for continued independence.
This mention validates the work undertaken by the medication management team, led by Tiziana Pelusi, and the PSW’s in ensuring continued success of this program.
