Help with the Health Care System and personal health matters. You will need a valid OHIP card. Provide nursing, personal support workers (PSW), social workers, physio therapists, assess safety of your residence, provide assistance in housing options.

Ontario Seniors' Dental Care Plan (OSDCP)
Coverage includes:
- check-ups
- repairing broken teeth and cavities
- x-rays
- removing teeth or abnormal tissue (oral surgery)
- treating gums, infection, paid and diseases
To qualify: you must be 65years or older, have a valid Ontario Health Card, meet income guidelines ($22,000 or less if unmarried, $37,500 or less if married)

Telehealth Ontario
Free and available 24-7 for ANY medical question. Call if your family doctor is on vacation and you require a prescription refill. This is a confidential service you can call to get health advice or information. A Registered Nurse will take your call will ask you to answer questions so they can assess your health problem and give you advice. Assistance is available in more than 300 languages.

Find a Physician
Register by phone with Health Care Connect and a nurse will search for a doctor or nurse practitioner who is accepting new patients in your community. To qualify, you need a valid Ontario Health Card and have NO family doctor.

With a recommendation from a registered physiotherapist or occupational therapist, rebates of 75%-100% may be available for walkers, wheelchairs, crutches, etc…

Lifeline is an easy-to-use personal response service that lets you summon help any time of the day or night – even if you can’t speak.

Seniors without Walls
Many older adults in our communities are socially isolated and unable to access in-person or online programs. Some are reluctant to meet face-to-face for health reasons. Others can’t because of physical or mental health issues, financial constraints, transportation difficulties, or technological barriers.
Seniors’ Centre Without Walls (SCWW) eliminates these barriers by offering free, telephone-based group programs that make it safe and easy for seniors to participate in interesting activities, meet new people, and make meaningful connections – all from the comfort of home.
